Admission criteria for geriatric rehabilitation in the Rehaklinik Dussnang.
Valid as of July 2022

Admission criteria apply to:

  • Patients after surgical treatment
  • Patients after conservative treatment
  • Conditions following an accident or acute illness or chronic progressive multimorbidity with potentially reversible deterioration of functional abilities or threatened loss of independence in basic and/or instrumental activities of daily living.

The following entry criteria must be met:

  1. Age >75 years (usually).
  2. Multimorbidity: simultaneous presence of at least three chronic or acute health problems
  3. Geriatric syndromes: simultaneous presence of at least three syndromes/symptoms such as:
  • Mobility disorder/immobility
  • Risk of falling/increased risk of falling
  • Cognitive impairment, especially resolving delirium
  • Incontinence of stool and/or urine
  • Depression or anxiety disorder
  • Impaired function of sensory organs (visual or hearing impairment)
  • Pressure ulcers
  • Polypharmacy and/or nonadherence
  • Chronic pain
  • Malnutrition and/or sarcopenia
  • Social isolation
  • Frailty/reduced resilience

 

Exclusion criteria:

Patients with the following conditions cannot be admitted:

× Medically unstable situations requiring acute hospitalization, e.g., unstable vital signs.
× Diagnostics not completed or imminent interventions/surgeries
× Acute, especially hyperactive delirium (requiring acute geriatric or geriatric psychiatric treatment) including patients with severe confusion and disorientation as well as uncooperative behaviour and a tendency to wander off.
In contrast, resolving delirium is not an exclusion criterion for geriatric (as opposed to organ-specific) rehabilitation.
× Patients with a lack of physical resilience due to severe impairment of cardiovascular/respiratory organs.
× Patients with last stage tumor disease
× Patients with a pre-existing severe need for care and with no improvement in symptoms.

  • Postoperative rehabilitation (area of gerontotraumatology, visceral surgery & vascular surgery).

  • Strength training for safe self-care and increased mobility
  • Fall prevention – balance and gait training
  • Building resilience for safe transfers
  • Pain management
  • Improvement of nutritional status after hospitalization or during illness
  • Increasing activity and participation levels and restoring body functions and structures
  • Helping patients achieve the highest possible level of independence in their living environment
  • Reduktion von Pflege- und Betreuungsaufwand
  • Improving patient outcomes and reducing care costs
  • Cognitive training

Admission criteria apply to:

  • Patients after surgical treatment
  • Patients after conservative treatment

Entry criteria (at least 4 to be met):

  • Necessary therapy intensity cannot be achieved on an outpatient basis
  • High risk of falling
  • Recurrent pain
  • Delayed mobility
  • Patient’s self-care in their home environment is impossible

Musculoskeletal rehabilitation services we offer:

  • Postoperative rehabilitation
  • Rehabilitation after conservative treatment
  • Prevention of falls – improvement of balance and gait
  • Targeted muscle development
  • Building resilience for safe transfers
  • Pain management and enhancing functional recovery
  • Building skills that patients need to achieve the highest possible level of independence in activities of daily living
  • Improvement of nutritional status after hospitalization or during an illness
  • Recovery of everyday functions following an illness or accident
